WHY IT COSTS MORE - Prior to the enactment of the March 2010 "Affordable Care Act," 85% of the health insurance sold in the United States reimbursed no more than 50% of the medical costs for "in-network covered benefits."  The ACA requires "bronze" plans to pay at least 60% of covered services, "silver" plans must pay at least 70%, "gold" plans must pay at least 80% and "platinum" plans must pay at least 90% (source: ACA).
